letters of recommendation - how to ask?

I've been lurking for a while now, and thanks for all the good info.

For those applying to multiple medical schools, how did you ask your letter writers to send it to many different schools, with many differnt requirements? I'm noticing some schools have forms to fill out, while others just give you an address to send letters to. Should there be a cover page for those with just an address?

Did you ask the prof. to write one letter, then give him instructions on how to send it to the different schools, and supply addressed envelopes, etc?

From what I've seen most schools prefer you send them LORs through a pre-advisory committee. I think most schools have one and you work things out with your pre-med advisor. The professors who write you letters of recommendation usually get a form as well and send everything to the advisor, who mails everything out for you. It really makes life very easy. I also found that all of my professors were familiar with the process and could pull out a rec in a day or two. I asked them over the summer by sending them a polite email. They usually responded back quickly and asked for a personal statement or something just to get an idea about my goals and such.
